- 5 O’Level credit passes (including English & Mathematics) in not more than 2 sittings
- JAMB UTME Result Slip
- SSCE Certificate or Statement of Result
- Birth Certificate or Age Declaration
- State/LGA ID
- Reference Letter
- Passport Photograph (plain background)
