Screening Documents (Originals)
Applicants are required to present the following original documents for screening:
- Completed Application Form
- Testimonial (Primary or Secondary)
- O’Level Certificate(s)
- All Higher Degree Certificates for Direct Entry (HND, OND, NCE, ICAN, etc.)
- NYSC Certificate / Exclusion / Exemption (Direct Entry only)
- Birth Certificate or Sworn Affidavit
- Marriage Certificate, Affidavit for Name Change, or Newspaper Publication (Married Females only)
Documents for Registration After Payment
After payment, applicants must submit the following documents:
- Application Form
- Admission Letter (Coloured & Laminated)
- Testimonial – Photocopy
- Student Information Form
- O’Level Certificate(s) – Photocopy
- All Higher Degree Certificates for Direct Entry
- NYSC Certificate / Exclusion / Exemption (Direct Entry)
- Online Generated Acceptance Letter & Biodata Form
- Online Course Registration Form approved by Course Adviser
- Handwritten Letter of Preparedness to Pay All Service Charges
- Reference Letter from Employer (Original)
- Reference Letter from Lawyer, Clergy, Imam, or Senior Civil Servant
- Online Student Payment Receipt
- Marriage Certificate / Affidavit for Name Change / Newspaper Publication (Married Females only)
- Birth Certificate / Sworn Affidavit
- Passport-size Photographs (4 copies; Name & Matric Number on reverse)
- JAMB Slip

- Applicants must attend screening within one month of admission notification; failure to do so may result in forfeiture of admission.
- UNILAG DLI does not use third-party agents. Beware of fraudsters.
